This happened when doing an auto-follow logic. I noticed the ES records cannot be inserted for any user who has logged on at least once since the Chatter feature is turned on. The error message is:
System.DmlException: Insert failed. First exception on row 0; first error: DUPLICATE_VALUE, duplicate value found: <unknown> duplicates value on record with id: <unknown>: []
